When you’re ready, tell me a bit about the student and what you’re hoping mentorship will help with, and I’ll follow up to talk through next steps. This application can be completed by a parent, guardian, or the student. That said, where it asks about the student’s own goals, challenges, and experiences, please answer in the student’s own words. It takes about 5 to 7 minutes to complete (a little longer if you’d like to upload materials).
